1099-Rs

Each year, Colorado PERA provides benefit recipients with personalized year-end tax information. This information is provided on IRS Form 1099-R, which is mailed to all PERA benefit recipients by January 31.

Payments Included on 1099-R

The 1099-R for 2019 reflects PERA benefit payments issued from January 1, 2020, through December 31, 2020.

1099-R Information Available Online

All benefit recipients can view and print 1099-R information from the PERA website by using a User ID and password. By logging in to your PERA account, you may access and print 1099-R information from 2003 through 2020.

Requesting a Duplicate 1099-R

Service or Disability Retiree/Survivor Benefit Recipient
You may request a duplicate 1099-R by calling PERA's Customer Service Center at 1-800-759-7372. Duplicates are printed overnight and mailed daily from February 1 through April 15. You may also view and print your past 1099-R forms by logging in to your account.

Refund or State Transfer 1099-R
You may request a duplicate refund 1099-R by calling PERA's Customer Service Center at 1-800-759-7372. Duplicates are printed on Mondays and Wednesdays and mailed on Tuesdays and Thursdays.

Short-Term Disability (STD)
The Short-Term Disability (STD) 1099-R is issued by Unum for members approved for STD during the 2019 tax year. You may request a duplicate 1099-R by calling Unum at 1-800-845-2290. Members approved for STD prior to 2012 should call Standard Insurance at 1-800-368-1135 to request a duplicate 1099-R.

1095-B Forms

Individuals will not have to make a shared responsibility payment if they were not covered for part of 2020. PERA is still required to mail 1095-B forms although you will not need them for your tax return preparation. You may also view your current and past 1095-B forms online by logging into your account.

Change of Address/Incorrect Address

Your 1099-R is still valid even if your address on the form is incorrect. Please notify PERA of any address change using one of the following methods:

  • Change your address by logging in to your account. (If a PERA benefit check is mailed to your address, you must submit your address change in writing and include your signature.)
  • Submit your change of address information by mailing or faxing it to PERA, or calling the PERA Customer Service Center at 1-800-759-7372.
