As you approach your 65th birthday, we know you have questions about transitioning to Medicare. The resources on this page answer many questions about enrolling in Medicare and selecting a PERACare Medicare plan.

To make this process easier, we suggest the following:

If you decide to enroll in a PERACare Medicare plan, follow these three simple steps:

  1. Enroll in Medicare Part B (and Medicare Part A if you are eligible to receive it at no cost).
  2. Submit the appropriate PERACare Enrollment/Change Form listed below to PERA prior to your requested effective date, but no more than 90 days in advance. You may submit your enrollment form online by logging into your PERA account (recommended) or you can download or order the form below. Please note, if you are not currently enrolled in a PERACare health plan, you must also submit a Certification of Previous Health Care Coverage form, which is also listed below.
  3. Once you receive your Medicare card, submit a Medicare Card Submission Form to PERA.
